The use of endoscopic third ventriculostomy (ETV) has recently been recognized as a beneficial alternative to patients being treated for hydrocephalus.

The procedure avoids all the inherent risks of implanted shunt devices including infection, breakage, bowel perforation, and shunt migration. "When feasible and safe, the use of ETV over shunting carries considerable benefit to the patient, especially when one factors in the cumulative complication rate with conventional shunting" says pediatric neurosurgeon Mark Souweidane, MD. Candidates for ETV have usually been selected based on the identification of adequate space in front of the brain stem on the preoperative MRI scan. In a recent article by Dr. Souweidane and co-authors, the use ETV is described for the first time in a series of patients who would have otherwise been considered ineligible and treated with shunt insertion. "We hope that with publishing this experience and describing the endoscopic procedure, more patients with hydrocephalus may enjoy the enormous advantage of ETV" says leading author Dr. Souweidane.
